Use/relationship: This DID will be used to identify materials that may contain toxic or
hazardous substances that are to be used, removed, or distributed during work operations and the
process/procedure for the removal or disruption of existing toxic or hazardous substances.
This DID contains the format and content preparation instructions for the data product generated
by the specific and discrete task requirement as delineated in the contract.
Requirements:
1. Format. The report shall be in contractor’s format.
2. Content. The report shall include the following:
a. Toxic or hazardous substance
b. Exposure monitoring
c. Method of compliance
d. Respiratory protection
e. Protective clothing
f. Housekeeping
g. Hygiene facilities and practices
h. Medical surveillance
i. Employee information and training
j. Signs
k. Recordkeeping
