This manual implements Air Force Policy Directive (AFPD) 91-2, Safety Programs, is consistent
with AFPD 13-5, Air Force Nuclear Mission, and provides weapons unique guidance to support
Air Force Instruction (AFI) 91-204, Safety Investigation and Reports. It directs procedures specific
to investigating and reporting United States Air Force (USAF) weapons mishaps and events. It
applies to commanders, managers, supervisors, and safety staffs at all levels, all persons who
investigate and report Air Force (AF) mishaps, and those persons who handle such reports. This
publications applies to all active duty, The Air National Guard, and Air Force Reserve military
and civilian personnel. This manual requires the collection and or maintenance of information
protected by the Privacy Act of 1974 authorized by Title 10 United States Code (USC), Section
9013, Secretary of the Air Force. The applicable SORN (FO91 AFSEC C) is available at:

processes prescribed in this publication are maintained in accordance with Air Force Manual 33-
363, Management of Records, and disposed of in accordance with Air Force Records Disposition
Schedule located in the Air Force Records Information Management System. Refer recommended
changes and questions about this publication to the office of primary responsibility (OPR) using
the AF Form 847, Recommendation for Change of Publication. Route AF Form 847 from the field
through the appropriate functional chain of command. This publication may be supplemented at
any level, but all supplements must be routed to the OPR of this publication for coordination prior
