This supplement implements and extends the guidance of AFI 91-204, Safety Investigation and
Hazard Reporting, 27 April 2018. It applies to all Air National Guard (ANG) personnel. This
AFI may be supplemented at any level, but all supplements must be routed to National Guard
Bureau Safety (NGB/SE) for coordination prior to certification and approval. Refer
recommended changes and questions about this publication to the Office of Primary
Responsibility (OPR) using the AF Form 847, Recommendation for Change of Publication.
Route AF Forms 847 through NGB/SE. For waivers to this supplement, submit an AF Form 679
to the ANG Workflow Organizational Box (usaf.jbanafw.ngb-cf.mbx.ang-
workflow@mail.mil). Ensure that all records created as a result of processes prescribed in this
publication are maintained in accordance with Air Force Manual (AFMAN) 33-363,
Management of Records, and disposed of in accordance with Air Force Records Information
Management System (AFRIMS) Records Disposition Schedule (RDS) located at
