This publication implements guidance in Air Force Instruction (AFI) 91-202, The US Air Force
Mishap Prevention Program, and DAFI 91-204, Safety Investigations and Reports. This
instruction applies to Air Mobility Command (AMC) Regular Air Force squadrons, operating
locations, and detachments assigned or attached to the 515th and 521st Air Mobility Operations
Wings (AMOW). It does not apply to the 43d Air Mobility Operations Group (43 AMOG), Air
National Guard, or Air Force Reserve Component units. Commanders are responsible for the
unit’s safety program. Ensure that all records created as a result of processes prescribed in this
publication are maintained in accordance with (IAW) AFI 33-322, Records Management and
Information Governance Program, and disposed of IAW Air Force Records Information
Management System Records Disposition Schedule. Refer recommended changes and questions
about this publication to the Office of Primary Responsibility (OPR) using the AF Form 847,
Recommendation for Change of Publication. The authorities to waive wing, and unit level
requirements in this publication are identified with a tier number (“T-0, T-1, T-2, T-3”)
following the compliance statement. See Department of the Air Force Instruction (DAFI) 33-
360, Publications and Forms Management, for a description of the authorities associated with
the Tier numbers. Submit requests for waivers through the chain of command to the appropriate
Tier waiver approval authority, or alternately, to the publication OPR for non-tiered compliance
items. This publication may not be supplemented or further implemented/extended. The use of
