Use/relationship:
The Equipment Inspection and Testing Report contains the results of all
equipment receiving acceptance inspection and testing and ESS and/or
qualification testing.
The report is used to evaluate accept/reject rate during acceptance
testing, ESS and/or qualification and corrective actions taken.
Requirements:
1. Reference documents: None
2. Format: Contractor format is authorized.
3. Content: The Equipment Inspection and Testing Report shall be
prepared from accept/failure data collected during equipment
qualification and/or production testing. The report shall contain the
following:
3.1 Title page that contains:
3.1.1 Report title
3.1.2 Equipment nomenclature
3.1.3 Report date.
3.1.4 Security classification of the report.
3.1.5 Contract number
3.1.6 Name(s) and official title(s) of the preparer (s) of the report
3.1.7 Signature of authorized company representatives
3.2 Brief introduction that includes the scope and purpose of the
report
3.3 A summary of equipment testing that includes:
3.3.1 Equipment serial number
3.3.2 Test name
3.3.3 Start and end time/date
3.3.4 Results (pass/fail)
3.3.5 Failure Analysis Report (FAR) number (for tests with failures)
3.3.6 Test procedure failure by paragraph number.
3.3.7 Cumulative operating time of acceptance testing and ESS
