3. DESCRIPTION OF PURPOSE
3.1. The Equipment Operational Log is used for recording equipment operation and maintenance action(s) required
during equipment operation for factory testing, evaluation, or as otherwise required.

7.1 This Data Item Description may be used whenever a Log type record of equipment operation and maintenance
action(s) is required.
7.2 UDI-T-23724, Failure/Malfunction Report, may be used in conjunction with this Data Item Description.

10. PREPARATION INSTRUCTIONS
10.1 The issue of the referenced documents in effect on the date of the invitation for bid or request for proposal form a
part of this Data Item Description to the extent specified herein.
10.2 The Equipment Operational Log shall provide blocks/spaces for operator recording of all data pertinent to
equipment operation, and the maintenance actions(s) required. The Log shall, as a minimum, provide the
following data:
a. Equipment nomenclature and serial number
b. Equipment operator (printed name and signature)
c. Date(s) of operation
d. Hour of operation
e. Equipment parameters
f. Purpose, i.e., burn-in test, reliability/maintainability test
g. Times of failures
h. Maintenance action(s) required, and equipment down time (and identification of failure/malfunction analysis
report if prepared)
i. Identification of part(s) repaired/replaced
j. Remarks/comments – for recording operator action(s), equipment status changes, maintenance
troubleshooting and repair action(s), assembly malfunctions symptoms, etc.
10.3 The Log may be in the contractor’s own in-house form and format, unless a specific format is required by Block
16 of the applicable DD Form 1423 data item.
10.4 Data to be recorded on the log shall be typed or neatly printed.
10.5 Abbreviations may be used for recording data, provided 1) they are authorized for use by MIL-STD-12C, or 2) a
list of abbreviations and symbols, with definitions, is provided.
10.6 Additional data shall be included when specifically identified and required by the DD Form 1423 or the contract
schedule/work statement.
10.7 Unless otherwise stated in Block 16 of applicable DD Form 1423 data item, multiple page Logs shall be
sequentially page numbered, and bound together in the most economical manner.
