SUBJECT: Mishap Notification, Investigation, Reporting, and Record Keeping
References: See Enclosure 1
1. PURPOSE. This Instruction reissues DoD Instruction (DoDI) 6055.07 (Reference (a)) in
accordance with the authority in DoD Directive (DoDD) 5134.01 (Reference (b)), cancels
Directive-Type Memorandum (DTM) DTM-07-020 and DTM-04-008 (References (c) and (d)),
and describes procedures in support of DoDD 4715.1E (Reference (e)) and DoDI 6055.1
(Reference (f)) to:
a. Update procedures for mishap notification, investigation, reporting, and record keeping.
b. Implement the Occupational Safety and Health Administration (OSHA) reporting
requirements in accordance with Executive Order 12196 and part 1960 of title 29, Code of
Federal Regulations (CFR) (References (g) and (h)).
c. Establish the DoD Mishap Data Requirements Working Group in accordance with DoDI
5105.18 (Reference (i)).
d. Establish requirements for interactions with the National Transportation Safety Board
(NTSB) and the Secretary of Transportation in accordance with sections 1131 and 1132 of title
49, United States Code (U.S.C.) (Reference (j)).
e. Establish requirements to report explosives and chemical agent mishap information to the
DoD Explosives Safety Board in accordance with DoD Manual (DoDM) 6055.09-M (Reference
(k)).
2. APPLICABILITY. This Instruction applies to OSD, the Military Departments, the Office of
the Chairman of the Joint Chiefs of Staff and the Joint Staff, the Combatant Commands, the
Office of the Inspector General of the Department of Defense, the Defense Agencies, the DoD
Field Activities, and all other organizational entities within the DoD (hereinafter referred to
collectively as the “DoD Components”).
